Your First Visit

Before arriving in our office for your appointment, please complete the appropriate registration package entirely, sign and date it, and bring it with you to your appointment. You will need to bring your insurance card and photo identification (such as a driver's license). You will also need to bring with you a referral form or electronic authorization number if your insurance requires these items.

If you have had tests in the past that are related to your current problem, please bring the results with you (lab work, hearing testing, etc.). It is also extremely helpful if you bring any recent x-ray studies (films and results) with you. You may sign these out at the radiology center where the study was performed.

We realize that your time is valuable and try our very best to stay on schedule. We ask you to assist us by being on time for your appointment (we request you to be 5-10 minutes early for first visits in order to process your completed registration. If you have not completed your forms, we request that you arrive 30 minutes prior to your appointment time). The most common reason for us to run behind is patient lateness. Please understand that we will need to reschedule your appointment if you are late for your appointment or if paperwork is not completed. Please cancel 24 hours in advance if you cannot keep your scheduled appointment so that your appointment time will be available for other patients. Occasionally emergencies may require our office to reschedule your appointment. We will contact you as soon as possible when this occurs.
